IMPORTANT: Please review, fill out, and submit this form to the best ability you can for your scheduled Confirmation. Not all questions on this form may apply to your Mass.

If you are celebrating as a single parish, celebrating with a few parishes in your family, or celebrating as a whole family, this sheet will allow you to fill out for all scenarios. It is not mandatory to work as a family, it is encouraged and supported. Thank you.

The Rite of Confirmation

Presentation of the Confirmandi- (to be done by the Pastor or Catechist): The names of those to be confirmed may be read aloud at this time.


**If the number of Confirmandi exceeds 25 then the Confirmandi will be presented as a group, not individually. All candidates stand. Bishop receives testimony from sponsors.
