All supporting documentation is required to complete this change. Failure to include all the necessary documents will result in the change not being completed.

Required documentation for Adding Minor household members:

• Completed Declaration of 214 Status form
• Social Security Card
• Birth Certificate
• Statement from current property owner confirming family member is allowed to move into unit
• Notarized letter from child’s parent designating custody and including expected length of stay (If Applicable)
• Court Awarded Custody Documents (If Applicable)

Required documentation for Removing Minor household members:

• Proof of new address for the family member leaving the household
• Statement from current property owner confirming family member has vacated the unit.
