The purpose of this form is to inform the UK Headquarters Scout Awards Team of Commendation Awards that have been awarded locally so they can be uploaded to a membership profile. This form is used to collect personal data about you and the individual receiving the Commendation Award.

The UK Headquarters Scout Awards Team will securely store (electronically) the details of these Commendation Awards for 3 months after the award has been uploaded to the membership system.
