Council Elections - Nomination and Consent

This form is used by Registrants who are seeking nomination for election to either District 1 or District 3 and replaces a manual nomination form. Submission of this form will launch an automated electronic process for seeking the approval of the nominators you identify. They will be emailed and asked if they support your nomination.

Section 1: Person Expressing Interest - Nominee

Please identify the District in which you are seeking nomination. Please note, if you hold a General Class Certificate of Registration your primary practice location must be in this district to be eligible for nomination. If you hold an Inactive Class Certificate of Registration, your home or contact address on file with the College must be in this district to be eligible for nomination.

Section 2: Nominators

In this section, you are asked to provide the name and contact information for two other Registrants who are in the same district in which you are seeking nomination who have agreed to nominate you for election to the Council. To be in the same district, the individual's primary practice location must be in the district if they hold a General Class Certificate of Registration or their home or contact address on file with the College must be in this district if they hold an Inactive Class Certificate of Registration. In the event that one or both of your nominators are not within your district, the College will attempt to contact you to seek other nominations provided time permits. If you submit your nomination at the end of the nomination period, there may not be sufficient time to contact you. It is your responsibility to identify two individuals from your district who will nominate you. Any errors in this regard are not the responsibility of the College.
