Use this form to request new or changed domain exemptions on the Grade 9-12 HS ELPA Summative. This form must be used for all domain exemptions to be implemented or updated between 3/15/22 and 4/29/22 (including both in-person and remote testing).

This form is necessary for students in grades 9-12, whether or not they are eligible for carrying domains via Honoring Student Proficiency on the HS ELPA Summative. This form is NOT necessary for students in grades K-8.

Once the domain exemption is entered and the student is ready for testing, you will receive a confirmation from ODE (at the email address you supply below).
