This application form is for candidates

EXTERNAL to Seattle Children's and UW Medicine


  • 12 full-day (6 hour) sessions held at Seattle Children’s Hospital. Includes didactics and hands on project work sessions. Scholars also present project updates at work in progress sessions.
  • Up to 45 AMA PRA Category 1 Credit(s)™ for physicians and up to 46 ANCC contact hours for nurses (live sessions).
  • Certificate of Program Completion.


Tuition: $5,000 due by September 1st. 2025

Preference is given to candidates with an established interest in quality improvement. Applicants must be able to commit time to attend monthly didactic and work in progress sessions. Strong applicants will have a QI project which is planned or already implemented prior to the program start date of October 2025.

1. Description of QI Research Project

(2 pages maximum excluding references, Arial 11-point, single-spaced, ½ inch margins – proposals not adhering to these instructions will not be reviewed). Please clearly indicate the stage of the project (planned, implemented / underway, completed).


Please include the following sections in your project proposal:


Brief background/significance: Why is improvement needed in your area of focus? What is the rationale for your intervention? Cite supporting literature - or lack of previous studies in this area.


Objective/Aim: What are you trying to accomplish with your QI project?


Time frame: what is the study time-period? Consider a pre-intervention baseline period, and post-intervention period.


Measures: List of potential process, outcome and balancing measures.


Potential data sources: Describe data sources for the proposed project –indicate ease of access to these data (i.e., electronic health record, chart review, manual data collection).


2. Letter from Department Head/Division Chief/Supervisor

Letter of support should acknowledge that tuition is $5,000 and due by September 1.


3. A half page personal statement

Indicating how participation in the program will benefit the participant. Limit to ½ page.
