Commissioning Requests must be received three weeks prior to the time Apex is to arrive on site. The 3 week lead time is necessary to review all system documentation, ensure that documentation is complete and accurate, obtain any missing information, as well as program the system, prior to arriving on-site. Apex will not schedule on-site visits, until all information below is verified as being complete. Please contact your Apex Project Coordinator to help you complete these forms if you have ANY questions.

If a Start-Up Request Form has been submitted and confirmed, and appropriate travel arrangements have been made (ie. flight, train, car rental, hotel, etc.) requestee will be held responsible for all associated cancellation fees. Apex will invoice purchasee all associated cancellation fees. Failure of back-charge payment will void all system warranty.

The EC's On-Site Contact will be responsible for signing the required Apex Field Engineers On-Site Visit Form. Please note, refusal of the On-Site Contact to sign the required Field Engineer's Visit-Form will void all Manufacturers warranties associated with the system and will prevent a Field Engineer from returning to the site for additional visits to complete the system. Also note that the EC is responsible for scheduling training with the end-user or their representative to take place during System Commissioning. For a project that has purchased only 1-visit commissioning this must take place the scheduled day of commissioning to avoid incurring additional visit fees.

In order to commission the Lighting Control System all of the below items must be complete prior to the date of the requested startup. If during the site visit access is limited to certain spaces or power is being interrupted in areas that are being programmed, additional costs may be incurred due to return visits.
