Information on this form is required to complete the appointment process for visitors to Stanford University. There may be additional information required to assist with a visa application process through the university’s Bechtel International Center. This information is being gathered through a secure online process and will be used only to complete Stanford internal administrative processes or for visa application process, if applicable. Intellectual Property (IP) - Stanford University resources are to be used solely for University purposes and not for personal gain or personal commercial advantage, nor for any other non-University purposes. Visitors to Stanford are required to complete an intellectual property agreement form (SU18A). The form can be viewed here: http://otlportal.stanford.edu/su18a .
Notes for visitors requiring a visa: As a condition of the visa visitors will be required to demonstration they have sufficient funds and adequate health insurance coverage to support themselves and any dependents while in the U.S. Schools may also have additional requirements for funding support and health insurance coverage. VSRs are eligible for Stanford's Cardinal Care and will be automatically enrolled unless declined.

Health insurance requirements: The United States Information Agency regulations require that all exchange visitors and their dependents maintain insurance at levels that meet the USIA-prescribed coverage.
