New Mentor Commitment Form

Thank you for your commitment to serve as a mentor for Big Shoulders Fund Scholars! We hope to connect over 400 scholars with 100 mentors each academic year and we are so excited that you have joined this vital program! Big Shoulders Fund asks that mentoring program volunteers commit to serving as a mentor for one full school year. Mentors are required to hold sessions with their mentees once a month. Mentor Agreements

I, as mentor, agree to: • Make at least a one-year commitment to mentoring; • Meet monthly with my students at their school (virtually); • Notify the principal if I have any concerns about my mentees; • Communicate with the students and their parents through the principal; • Accept assistance/guidance from my mentee’s teacher, principal, and/or school support staff; • Ask school principal or Big Shoulders Fund staff when I need assistance, do not understand something, or am having difficulty with my mentoring relationship; • Notify Big Shoulders Fund of any changes in my contact information; • Notify Big Shoulders Fund of any significant change in my mentees including any financial/tuition challenges they may experience; • Provide students with positive feedback/closure to our mentee/mentor relationship whenever it comes to an end; and • Refrain from contacting or seeing my mentee outside of the established parameters and school sites (or scholarship receptions, if applicable) where the program takes place.
