Cook County Government Seeking Community Engagement & Outreach Partner


For consideration, please complete the following questions. Be sure to answer each question completely. Incomplete applications will not be considered.


When beginning your application, be sure to look at the end of this application and have the list of attachments prepared to upload.


Deadline: All responses must be submitted no later than Wednesday, February 28, 2023, by 5:00pm Central Time. All applications must be submitted electronically. Late submissions will not be considered.

Section III: Organization Overview

Please provide an overview of your organization’s history, qualifications, experience, current resources, and accomplishments related to administering and providing community engagement and outreach services, especially survey development & canvassing efforts across diverse communities.

Section IV: Survey Experience Overview

Describe your experience in designing culturally competent survey tools and developing data analysis processes. At a minimum, this description should address the following: topic area of past survey tools developed, volume of respondents, method used for collection, size of data collection team, approach to data analysis, and project budget as well as project duration.

Describe your experience developing public-facing dashboards, preferably designed for website integration. You may attach a maximum of (2) examples as part of the supporting documents.

Please describe your data collection plan for engagement and outreach within Suburban Cook County. The plan should include details regarding any general marketing to all levels of stakeholders, including residents, organizations, businesses, institutions, and other stakeholders. Please highlight any targeted marketing strategies to specific communities in the region, including door-to-door canvassing, telemarketing, newspapers, radio and television, fairs and community gatherings, social media, etc. This strategic plan should explain why the proposed approach will be effective. Please include an estimate of how many individuals will successfully complete the survey, broken down by category of method used (door-to-door canvassing, online survey tool, phone, etc.).


You may submit up to (2) examples of print/digital marketing materials developed by your organization as part of the supporting documents.

Section V: Management Capacity

Describe your organization’s experience managing teams and projects similar in scope and nature to that which is being proposed. This description should include your approach and organizational capacity to quickly design an effective survey tool and begin collecting data across Suburban Cook County through multiple communication vehicles. This includes, phone, door-to-door canvassing, townhalls, and by other means.

Identify your key staff members (e.g., management, staff responsible for the successful provision of services), their project roles, and their requisite experience in providing community engagement and outreach services. Please provide staff resumes to substantiate this narrative as part of the required attachments.

Are you applying as a sole agency or will you have a teaming partner? Please include a description of which portion(s) of the services will be subcontracted and the name and address of the potential subcontractor.

Section VI: Budget and Reporting

Please discuss how your organization will monitor program expenditures. Include a description of your organization’s fiscal monitoring procedures and any experience your organization has with federal and/or local government fiscal compliance requirements.

Please discuss any experience your organization has with federal and/or local government grant reporting requirements.

Please describe how your team tracks and provides data and metrics – especially sensitive data with personal information – for monitoring, and reporting. How will this data be accessed by the County?

Past Performances*: Submit the proposed Project Team members’ past performance on at least three (3) specific projects relevant to this request. Greater consideration will be given to past projects with stronger relevancy and on behalf of city, county, state, or other government entities.


501(c)(3) IRS Determination Letter: If applicable, applicants should submit a copy of the IRS Determination Letter or Affirmation Letter exhibiting that the Corporation is tax exempt under 501(c)(3) and 501(c)(4).


List of Board of Directors (If applicable): Please submit a list of your Board of Directors.


Copy of Articles of Incorporation (recommended): Please submit a Copy of Amended Articles of Incorporation.


Certificate of Good Standing (recommended): For applicants in the State of Illinois, please submit a Certificate of Good Standing. For applicants outside of Illinois, please submit a certificate of similar state documentation.


Most Recent Financial Statement or Audit*: Submit your A133 Single Audit, if applicable. Otherwise, submit your latest audited financials. If you do not have audited financials, you may submit other financial documents for consideration.
