The Friends of ADAMH was established to recognize an individual or group/agency who embodies the vision and values of the Fairfield County ADAMH Board. The Board's vision is to create opportunities for Fairfield County residents to be healthy members of their communities. The values of the ADAMH Board are; Accountability, Adaptability, Agility, Collaboration, Cooperation, Data Driven, Diversity, Evidence-based, Fiscally Responsible, Impartiality, Objectivity and Responsiveness.

Please submit a narrative no longer than one page describing why the individual, group, agency or organization should be recognized for the award by including specific ways the nominee exemplifies the criteria for the award. If mailing, print, complete, and mail to Fairfield County ADAMH, 108 W Main Street, Lancaster, OH 43130. If emailing, send to jhoward@fairfieldadamh.org. All nominations must be received by March 21, 2025. If nominating more than one individual, you must submit a separate form and letter of recommendation for each nomination. Nominators whose nomination is selected will be video recorded prior to the event and the video will be played the evening of the annual dinner.

Awards will be presented at the Annual dinner - Creating Hope and Breaking the Stigma on Tuesday, April 29, at the Crossroads Event Center in Lancaster, OH.
