Computer Workstation Evaluations

1.    Complete the UCSF Health Ergonomics Training module in UC Learning.

2.    Review the photo instructions and attach them to the request form.

3.    Submit an ergonomic evaluation request form via this link.

4.    An evaluator will contact you to schedule an ergonomic evaluation. Methods may be via Zoom, phone call for troubleshooting, or in-person.

Due to the current volume, UCSF Health Ergonomics will contact you in approximately 4-6 weeks to discuss scheduling.
