Advancement Services Request Form
There are two departments within Samford University that submit grant proposals to foundation and corporate sources—University Advancement (UA) and the Office of Research (OR). All proposals to external funding entities must be submitted in partnership with one of these offices.

Standard requests follow typical processing times.
Please allow at least 72 hours for your request to be completed.
Please allow a minimum of 2 weeks to fulfill your request if possible.

A stewardship contact can be utilized on reports if the financial contact for an organization and the stewardship contact are not the same person. Please provide the SUID of the organization below in addition to the stewardship contact name. If available, the SUID of the stewardship contact is also helpful.
If you need a preliminary count of the individuals on a list ahead of time, please also complete a separate request for "Count" with your criteria and the date for which you will need the count.
Please indicate the donor and timeframe you are requesting a giving statement for. Giving statements include: gift amount, gift designation, and gift date for each gift made by the donor in the timeframe.
Note: Advancement Services provides all donors who give at least three gifts that total $5,000.00 or more within a calendar year giving statement by January 31st each year.
Please provide the SUID for the record to be updated and select the appropriate areas to be updates from the below list.
Please check FPM, Gift Reports or Banner Self-Service before submitting a gift inquiry request. Gifts given online via GiveCampus may take 2+ days to be received by gift processing and posted.
