Exception to Academic Policy

The petition process is meant to accommodate students with extenuating circumstances or emergencies beyond their control that may impact their academic records. Examples may include but are not limited to medical/mental health circumstances and those of your family members, death of a family member, change in work schedule as mandated by your current employer, etc. Students should be able to show that the circumstances were beyond their control and that they occurred during the semester for which they are petitioning. We encourage students to include any documentation that may support their petition. This form is not meant for academic exceptions (ex. bypassing course prerequisites).

Attach supporting documentation to verify your extenuating circumstance. For example, documentation from your medical provider, obituary or funeral program, deployment notification, letter from your employer, etc.
