Nationwide Children's Hospital (NCH) is offering school required immunizations to incoming kindergarteners, 7th and 12th graders this Fall. Flu vaccines and other overdue immunizations are available at any grade level as well.

If you are interested in having your child(ren) receive their immunizations, please complete this interest form. If you have more than one student, please complete a separate interest form per child.

You will receive a phone call from the NCH clinic staff to confirm and answer any questions prior to the clinic day. Parents or guardians are welcome to join immunization appointments, but not required to do so.

These services will be billed to your insurance. If you do not have insurance, vaccines may still be given. A staff member will follow up with you to collect more information.
