Officer Wellness and Mental Health Grant Program - Final Impact Report

Law enforcement agencies receiving Officer Wellness and Mental Health Grant funds are required to submit a final impact of funding report and expenditure report to the Board of State and Community Corrections (BSCC) by December 1, 2025. The purpose of collecting this information is to demonstrate the impact these funds made on improving officer wellness. These reports will be shared with the California Commission on Peace Officer Standards and Training (POST).

Please respond to each of the questions to complete your final impact report. It is recommended that you prepare your narrative responses in advance in a separate file and copy and paste the responses into the form, as you cannot save the information in the form and return to it at a later time. Forms must be submitted by December 1, 2025.

If you have any questions about completing the form, email the BSCC at officerwellnessgrant@bscc.ca.gov.
