Your application must be complete, with no sections left unanswered. Becoming a Mentor is both and honor and a great responsibility. It is also an opportunity to strongly impact someone’s career and working life.

This information will be used, in part, to match you with the best Mentee. There will be several opportunities for you to get to know each Mentee and provide your input into which Mentees would be the best fit for you. The information collected in this application will not be shared with anyone outside the Mentorship Team Members; including the Director, Co-Director, Program Manager, Coaching Circle Leader, and Communications-Marketing Manager. This information may also be reviewed by PMI-Phoenix Board Member(s) involved in or leading our Mentorship Programs. All applicants and Mentorship Team members at PMI-Phoenix will be required to sign an agreement, as well as review the PMI Code of Ethics & Professional Conduct.
