IRS Classification Request

Consultants, Speakers*, Interpreters, Game Officials and other professionals are collectively known as Independent Contractors (ICs). Independent Contractor is an IRS classification of worker and federal guidelines dictate how to determine if workers should be assigned as ICs or employees. Failure to properly classify workers can result in significant penalties.

This form is used to determine IRS classification. For detailed instructions on completing the form, see the IRS Classification Request Companion Guide.

*Speakers who are only being provided the cost of travel are not considered Independent Contractors. Please refer to the University Travel Policy 6180 for details.
