DisGo Ride Request Form

Please complete this form if you need a single ride(s) or rides on a repeating basis throughout the quarter (e.g., on Monday/Wednesday/Friday).

We will try our best to accommodate the time that you have requested; however, we reserve the right to modify your time. In the event that we need to modify your time, we will call you to confirm this change. If you do not have a phone number, you will be contacted through your e-mail address.

Please note that the DisGo Service is available Monday through Friday, 8:00 a.m. to 9:00 p.m. No service on weekends, university holidays, or during winter break.

Please review the following conditions for DisGo service:

• If you request a ride after 7:00 p.m. and do not receive a confirmation e-mail, do not expect rides the following day.
• If you need to make changes to an existing ride, please call at least 60 minutes prior to your ride at 415.404.1059.
• Due to scheduling constraints, drivers are instructed to wait no more than five minutes past the pick-up time.
• If you are not present for any ride, your remaining rides for that day will be canceled. If you still need your remaining rides, you must call to confirm those rides.
• Stanford Transportation and DisGo reserves the right to refuse service.

Please provide the specific details of your ride request. If you are requesting more than six rides, please complete a second ride request form. Please submit rides in chronological order.

Please be specific as to your pickup location by using street names or building numbers. Specify the building number or name as well as the street you will be facing, such as Building 420, inside the Quad or Mirrielees, facing Escondido Rd.
