Phase III Credit Requests must be submitted at least 90 in advance of the start of the rotation (for UNM courses at least 45 days). Submission of this request is not a guarantee of approval, and you should check in w/ OMSA before you make any non-refundable travel arrangements. We cannot approve any credit requests outside of UNM until an Affiliation Agreement is finalized. You will receive a final approval email when your request is approved. If you make lodging arrangements (rentals) *before* the credit request has been approved – that is at your own financial risk.


*No two Phase II or Phase III courses may overlap, even by one day*


If your credit request is not approved before your rotation begins, you will not be able to earn credit for the course and you will not be covered by UNM's malpractice insurance.


If you have not received a Notice of Approval from Smartsheet, your rotation has *not* been approved. It is your responsibility to ensure your rotation is approved prior to starting. Please contact OMSA if you have not received approval and ask for an update on the status.


Note: We have seen some agreements take more than 90 days depending on negotiations and approvals between the legal teams of the host and home institution. While it is never our intent to deny a credit request, we cannot approve any requests without fully executed agreements.

This is the faculty member who will be supervising and evaluating you on this rotation. They must be an MD or DO. They will need to approve this request once you submit it before it can be approved by the Phase II Clerkship/Rotation Director. The request will come via auto email from "Smartsheet."

Be specific about the days and hours you will be working-for example:


Rotation will include outpatient and inpatient medicine. Students will spend 4-5 days per week (approximately 40 hours) x 2 weeks working with an attending seeing patients in the outpatient setting. Students will see patients presenting with acute illnesses as well as routine physical exams.


Students will spend 2 weeks in the inpatient setting. Patient care responsibilities include admission histories and physicals, following patients through the length of their hospitalization, preparing patients for discharge, and completing tasks (documentation, writing orders, discharge planning) that other interns on the team will be doing.

If you are requesting ICU or Sub-I credit and the course description does NOT meet UNM's requirements, the request may still be approved for regular clinical elective credit.

A UNM Dept. Director/Chair must approve your credit request. For example, if your desired rotation is in Internal Medicine the phase II IM clerkship director needs to approve. If you are unsure of who should approve your request please contact hsc-omsa@salud.unm.edu.

If the proper forms, information or permissions are not submitted or not submitted before the start of the rotation, I will *not* receive phase III credit for this course and I will *not* be covered by UNM SOM's malpractice insurance.
