Each angler must submit this form once. If you're submitting information for four anglers, complete the form four times but be sure to send them the waiver link separately.

If you are the team Captain and don’t have the participant details yet but want to order four XL shirts (for example), just send an email with the shirt sizes to michelle@ampartner.net. However, please submit the forms once you have the necessary information. This form can also be submitted if you have a change in anglers. Just make a note in the "NOTES" section.
