Transfer Requests

A transfer request may be submitted only for households who were determined and documented as chronically homeless at the time of enrollment into their current program. All transfers are dependent upon availability in PSH programs. Simply submitting a transfer request is not a guarantee that the transfer request will be approved.


Once submission of this request is completed, the transfer committee will meet the following Monday at 4 PM. It is required that the staff submitting this request is present at the transfer meeting to answer all questions the transfer committee has. Failure to be present will result is your transfer request not being discussed by the transfer committee.

 

For transfer to be considered, all of the following must be attached here: proof of chronicity, IDs, social security cards, verification of disability.
