Program Verification Form Request

Students applying for certification in Delaware, please visit Delaware Educator Data System (DEEDS) for instructions.

Students applying for out of state certification, please complete the form below.

Program Completion/Verification forms required for state licensing/certification must be reviewed and confirmed by the program chair, then approved by the college dean. Following those approvals, the Wilmington University Registrar applies the official university seal.

The original, signed and sealed form will be mailed to the address you provide below. An electronic copy will also be sent to your email. Please allow at least two weeks for the forms to be completed.

Note: Your form will not be completed and sent to you until your degree is conferred and posted on your transcript.
