2025 Health Care Mutual Rebate Program

GACCP members that carry Health Care Mutual workers’ compensation coverage are eligible for a 70% rebate on your membership dues!

For current, active members that already carry Health Care Mutual workers’ compensation coverage:

Complete this submission form. You will be required to upload images of the required documents via either mobile device or computer and have a valid and accessible email address.
You will receive a confirmation email from director@gaccp.org that your rebate submission has been received.
Once your submission has been reviewed and approved, your rebate will be issued in the method requested in the submission form.

Rebate Instructions

Rebates will be issued in the same manner as the original membership dues were paid. An alternative rebate payment method may be provided.

Terms and Conditions

To receive your rebate, you must satisfy each of the rebate requirements and provide the following documentation:

(A) be a current, active member for the 2024 membership year of the Georgia Association of Community Care Providers and not have any outstanding balances due to the association;

(B) be a current participant in the Health Care Mutual Worker’s Compensation coverage providing confirmation of the insurance policy.

Failure to follow each of these steps is a rejection of this rebate offer. Offer valid only for the 2024 membership year and will be re-evaluated for the 2025 membership. Membership is prorated during the year so the rebate amount is 70% of the membership dues paid, and not necessarily the entire membership dues amount promoted on www.gaccp.org. Allow up to 6 weeks to receive the rebate. GACCP reserves the right to cancel, suspend, or modify part of or this entire rebate program at any time without notice, for any reason in its sole discretion. Limit one rebate per company membership.
