Motiva® by Establishment Labs® Gold Card Program Terms & Conditions

This document describes the terms and conditions of Motiva® by Establishment Lab’s® (“Motiva”) Gold Card Program. Participants are eligible for the Gold Card program if they are at least 22 years old, a legal resident of any U.S. state or Puerto Rico and have undergone breast augmentation surgery with Motiva Implants® in the United States or Puerto Rico between 10/1/2024 and 12/31/2025 in which two (2) Motiva® breast implants are implanted during the procedure (the “Program Eligible Participants”).

Program Eligible Participants may receive a $50 prepaid gift card (“Gold Card”) from Motiva® for making one post relating to their personal experience with Motiva Implants®. The post must meet the following requirements:

(1) be posted on a Program Eligible Participant’s personal, but public Instagram or TikTok account;

(2) tag @Motiva_USA (or other designated social media handle by Motiva®); and

(3) tag the Program Eligible Participant’s implanting Surgeon.

Even if a Program Eligible Participant has met the above requirements, Motiva® may reject a post if it is not consistent with Motiva®’s messaging, as determined by Motiva® in its sole discretion.

To redeem the Gold Card benefit, Program Eligible Participants should visit www.MotivaUSA.com/GoldCard and submit their Motiva Implant® information (including serial numbers of the implants) along with a link to their social media post for Motiva’s review and approval. Once approved, Motiva will send the Gold Card to Program Eligible Participants within sixty (60) days. Additionally, after providing approval, Motiva will require each Program Eligible Participant to add the statement “#ad” to their post before they can receive the Gold Card.

Gold Cards are subject to limited availability and only available while supplies last. There is a limit of one (1) Gold Card per Program Eligible Participant. Motiva reserves the right to cancel this program for any reason, at any time, without notice. Gold Card program is void where prohibited or restricted by law.
