Intent of Form:

The Program Director must complete this form for any in-person or online program/activity involving minors at the San Bernardino or Palm Desert campuses, or any offsite CSUSB-affiliated program/activity.

Instructions:

The Program Director (or their designee) plays a crucial role in the registration process. They are responsible for registering the program with the Office of Risk Management in advance, ensuring all requirements outlined in this procedure are met. Registration should be submitted at least 30 days before the event/program.

The sponsoring department must retain all supporting documents for at least three years following the activity or until the minor participant turns 20, whichever is longer.

CSUSB’s Youth Protection Program covers individuals under the age of 18 who are not matriculated university students.
