QuickMAR: Archives/Cancellations
All Cancellations require 30 days notice. The facility will be contacted by the QuickMAR team for confirmation of the cancellation.
If you do not provide facility contact information or upload documentation from the facility confirming the archive we can not process this request.
Do not put in the Pharmacy code from the dispensing system- Please enter the QuickMAR ID from the QuickMAR system
THIS MUST BE PROVIDED OR THE REQUEST CAN NOT BE PROCESSED
Please list any information we need to know about this cancellation request. If this is a request to cancel a single cart please put that information in the comments below with the Pharmacy Code that needs to be removed.
This is required for all cancellations. If this is not attached the QuickMAR team will obtain this documentation but it could delay the cancellation request.
