This survey is intended for Phase 2 organizations in Clatsop County that identify as critical workers and wish to register as a group. "Critical workers" are defined as those who work in industries essential to the functioning of society and environments with a substantially higher risk of exposure. Other Phase 2 groups include people of all ages with underlying conditions, those live/work in congregate settings such as detention centers, and the general population. If you represent a critical worker organization that has asked employees to register themselves or a Phase 2 citizen who doesn't identify with an organization, please use the "Phase 2 Vaccination Interest Form (Individuals)" survey found here: https://app.smartsheet.com/b/form/3c45e89896cd4d85acc777af191ead6c As we move through the different phases of distribution and rollout, the Vaccination Task Force will reassess needs and prioritize based on the latest guidance received from OHA. We do not know yet when vaccines will be made available to your organization, but your help and partnership at this time is extremely valuable in assessing needs. We will use the information you submit here to notify you when you are eligible to receive a COVID-19 vaccine and will also send vaccine updates as they are available.
