Welcome to the district-funded student-employee hiring eligibility verification form.


Before completing the form, please read the following information:


  • This form is for hiring district-funded & Federal Work-Study student employees.


  • Any student employee with access to cash, work where theft or loss is a risk, or interact on their job with children under 18 must be fingerprinted before they can work, as must any student with a criminal conviction. If that requirement is checked on the form, the Student Employment Representative will email the necessary paperwork to the student and must receive the completed assessment before the student can be authorized to work.


  • Any student employee who will interact on their job with children under the age of 18 is required to complete FHDA’s TB Assessment within 30 days before employment. If that requirement is checked on the form, the Student Employment Representative will email the necessary paperwork to the student and must receive the completed assessment before the student can be authorized to work.


  • International Students are required to have a SSN before eligibility is done and clearance from the International Students Office. Please contact us if you need help with this process at fhstudentemployment@fhda.edu

Student Employee Supervisor Information

 

The supervisor should be the staff member that approves the student timesheet each month.


Please note that TEA employees may not approve timesheets. A TEA may be the primary supervisor, please also provide a secondary approver who is a permanent employee as well. We will flag the TEA employee for FYI in banner, you will be able to view the timesheet in banner, Please let the secondary approver know if there are any issues with the timesheet that need correcting.
