In order to ensure the health and safety of all our patients and staff, Hamaspik HomeCare is requiring that all staff complete the below form PRIOR TO THE START OF YOUR SHIFT. Staff is also required to monitor their temperature twice a day. PLEASE NOTE: All staff are now required to wear a face covering when within 6 feet of the patient. If you don't have a face covering, please reach out to your coordinator.
