This form is required of all international students who are transferring to another school in the U.S. Follow the instructions below to complete your transfer out request.
Instructions:
1) Complete this form and upload a copy of your acceptance letter to your transfer school as a pdf in the relevant section at least 5 business days before your planned transfer date.
2) Do not submit the transfer request until you’ve made a final decision about your transfer. Once your SEVIS record has been released to your new school, the ISP office will no longer have access to your SEVIS record.
3) Clear your Skyline College account if you have any balances.
4) You must stop any on campus employment, CPT employment or OPT employment by your SEVIS record release date.
5) If you are traveling outside of the U.S. after the SEVIS release date, you must re-enter the U.S. with your new school’s I-20.
