Clinical Trials Questionnaire

Projects that meet the criteria for being considered a clinical trial are required by NIH to submit full clinical trials documentation including registering the project on ClinicalTrials.gov. Because of these requirements, we strongly encourage proposals that center on secondary data analysis and/or qualitative data collection.

  • Project proposal, which should be no more than 3 pages in length (excluding references, appendices, and the required supporting materials listed below).
  • Letter of support from the sponsoring mentor
  • NIH Biosketches for the applicant, sponsoring mentor, and any other Key Personnel or Other Significant Contributor
  • Detailed Budget for Budget Period (Form PHS 398, Page 4).
  • Budget Justification (using PHS 398 Continuation Page).
