Dear student's parent or legal guardian,


Please complete and submit this form if you are requesting a letter stating that your child is participating in a Community Eligibility Provision school where breakfast and lunch are offered to all students at no charge.


If you have any questions or need further assistance, please contact the Meal Benefits Department by email at meal.applications@ocps.net or by phone at (407) 858-3110 ext. 329-3966.

As the parent or legal guardian of the student, please type your email address, which will be used to email you the requested letter once verified.
