Submit one form per event and complete all required sections to avoid processing delays. *Forms submitted before 5 pm CDT, during the meeting, will be processed the same day and available in the meeting agenda, within the platform by 7 am CDT the next day. **Forms submitted after 5 pm CDT will be available in the meeting agenda within 48 hours.
