The National Association of Chronic Disease Directors (NACDD), in partnership with the Centers for Disease Control and Prevention (CDC) developed a series of brain health educational materials for use by healthcare practitioners and public health professionals.

CDC BOLD and NHBI awardees are encouraged to share messaging resources whenever possible to reduce duplicative efforts. NACDD has granted permission for you to add your logo to these materials.

The process outlined below applies to all modification requests including BOLD grantees, non-profit organizations, HBI grantees, Public Health Centers of Excellence, and non-BOLD public health departments. Depending on your needs, please note this guidance:

TO ADD YOUR LOGO TO A MATERIAL:

1. Complete this form to initiate a request.
2. Include your high-resolution logo, minimum 300 dpi.
3. You will be notified via email by NACDD when the modifications are complete.
4. The CDC and NACDD logos are fixed and cannot be removed or changed. Your logo will be placed underneath.
5. CDC approval is not needed.

TO REVISE CONTENT/IMAGES FOR CULTURAL APPROPRIATENESS:

1. Complete this form to initiate a request.
2. If you would like to revise multiple products, submit only one form for all materials.
3. NACDD will contact you via email to discuss potential edits to images and/or text.
4. Once NACDD approves the request, the draft materials are sent to CDC for approval.
5. Edits to text and/or images require approval by CDC and may take approximately 3 to 4 weeks for review.
6. You will be notified via email by NACDD when the modifications are complete.
