EAG Working Group Interest Form
EAG Working Group Interest Form
Thank you for your interest in participating in the Evaluation Advisory Group (EAG) for the Public Health Infrastructure Grant (PHIG) evaluation!
The EAG brings together individuals and organizations who will actively use the evaluation findings including PHIG recipients, health department staff, CDC staff, national public health organizations, community partners, and affiliated members. EAG Working Groups (WGs) serve as a collaborative mechanism for reviewing, recommending, and guiding National Evaluation sub-evaluation activities alongside the National Evaluation Team.
Each WG will provide regular input, feedback, and guidance to the evaluation team responsible for its associated sub-evaluation. The National Evaluation Team (NET) will facilitate meetings and engagement. WGs will be launched and members will be invited on a rolling basis beginning in early 2025. For more information about EAG WGs, see the NET EAG Overview.
Please note that the EAG WG participation is voluntary. An ideal WG candidate should be aligned with the following criteria:
- Have firsthand experience with PHIG and the sub-evaluation topic the WG seeks to address, or a genuine interest in affecting change in this area.
- Be comfortable representing their organization, agency, peers, and/or communities within the WG.
- Commit to attending monthly meetings, reviewing materials outside of allotted meeting times, participating in WG activities, and advancing the WG’s collective goals.
- Dedicate 1–3 hours per month to WG activities, ideally remaining engaged for the duration of the sub-evaluation (expected to last 18–24 months).
The NET also anticipates opportunities for EAG members to contribute on an ad-hoc basis by reviewing evaluation deliverables and providing insights into other NET activities. This form includes an opportunity to indicate if you would like to participate in ad hoc engagements as well.
Please complete the form below to indicate your interests in upcoming EAG activities. Submitting your interest is not a formal commitment but enables us to share relevant details and next steps as opportunities emerge.
