Enrollment Appeal - Former Undergraduate Student

INSTRUCTIONS
Please read carefully the information and instructions below before proceeding to complete the form. The University takes requests for exceptions seriously and will consider exceptions to enrollment policy involving extenuating and unpredictable circumstances and when accompanied by supporting documentation. In accordance with university policy, students who register for a course at MSU are responsible for managing their registration and following appropriate enrollment procedures and deadlines, including meeting with their academic advisors, and terminating course(s) or semester registration if discontinuing enrollment at the university. If the student is unable to submit, a designee must contact the Office of the Registrar on their behalf.
PURPOSE OF REQUEST
This form is for former undergraduate students who wish to request an exception to one of the following enrollment policies:
- Last day to cancel all term registration.
- Last day to drop course(s)
- Last day to drop course(s) with a grade of "W"
- Last day for withdraw for the term.
This is in accordance with the Academic Catalog and the Academic Calendar.
SUBMITTING SUPPORTING DOCUMENTATION
Exceptions to enrollment policy are coordinated by the Office of the Registrar and reviewed by the Exception to Enrollment Policy Committee. Follow the instructions below for the selected exception option. Students are responsible for preparing and submitting:
- The exception request form, including a short description of the circumstances.
- All supporting documents from corresponding Third-Party source(s). Examples of third-party documentation include: military orders (DD214), doctor's note, emergency incident report and an official academic transcript from the school you attended instead of MSU. The supporting documentation can be: 1) Attached to this form; OR 2) Emailed to enrollment-appeal@morgan.edu; OR 3) Mailed to Office of the Registrar, 1700 E. Cold Spring Lane, Tyler Hall, Baltimore, MD 21251; OR 4) Dropped off to the Bear Essentials OneStop in Tyler Hall main level.
- All requests must be accompanied by a statement from the instructor(s) to report last date of attendance, last date of participation, and/or last date of use of university resources for the course(s) in question. This information is required for all requests, under all circumstances. The course instructor(s)-of-record should send an email directly to enrollment-appeal@morgan.edu. This information and the supporting documentation (see examples above) will be reviewed by the Committee.
REVIEW AND NOTIFICATION
Incomplete appeals will not be reviewed by the Committee. Failure to follow through on any further instructions or requests by the Committee for additional information within two calendar weeks will result in the exception request becoming void. Follow up correspondence should be clearly labeled “Exception to Enrollment Policy” and should include student name and ID number. You can generally expect a decision via e-mail within 30 working days from the time the COMPLETED exception request, provided that all necessary documentation are received by the Office of the Registrar. All decisions are final.
FINANCIAL AID IMPLICATIONS
Students who have received any type of scholarship(s) and/or financial aid (including loans) may lose eligibility based on an enrollment exception approval. The reduction or cancellation of financial aid and/or scholarship(s) as a consequence of an enrollment exception approval may result in a larger balance owed to the university. You are strongly encouraged to meet with a financial aid counselor PRIOR to submitting an enrollment exception request to discuss the potential impact an enrollment exception approval will have on your awards. Once an enrollment exception request is approved, it cannot be rescinded even if the balance owed to the university is higher than the current amount due.
