Compliance & Ethics Hotline Reporting Form

The purpose of this hotline is to provide a confidential means for CIA students and employees to use -- anonymously if desired -- to report incidents or issues of concern, so that the appropriate persons may investigate them, determine if allegations are substantiated, and if so, promote resolution.

The CIA has a range of reporting channels - is this the right one for your concern? Use this communication channel to report issues other than harassment, discrimination and sexual misconduct (please use the Title IX hotline for those).

If your concern is about grades or the academic behavior of another person, please try discussing it with the appropriate dean before using this tool.

Use this form to report: conduct that is known or suspected to be illegal, unethical, against CIA policy or procedure, or in violation of accreditation or other standards governing the CIA's operations. All reports are confidential. You may report anonymously. CIA policy prohibits retaliation against persons making reports in good faith.

If you have any questions about which form to use, please contact the Title IX Coordinator and Legal Advisor at (845) 451-1614 or CIAHotline@culinary.edu. Thank you.
