If equipment is assigned more than two people, please attach a document with who the additional equipment needs to be assigned to.
If software is assigned more than two people, please attach a document with who the additional software needs to be assigned to.
(i.e. increase in revenue, improvement in customer satisfaction, reduction in costs, increase in productivity)
Once your Area VP approves, if applicable, an approval request will be routed to the ISO and / or CIO for review and approval. If the purchase is over $100,000, an approval request will be routed to the Vice-President of Administration and Finance.
Please note - if your purchase is equal to or greater than $25,000 please make sure you have consulted with ASU Purchasing regarding appropriate purchasing methodology PRIOR to submitting this form.
If your Director, Dean, or Chair is the same as your Area VP, select 'No' for this option. Otherwise, they will receive multiple approval requests, which may delay the processing of your EDP request.
