Request for Alameda County to Participate in Your Health Fair!


Alameda County Health participates in nearly 100 health fairs across Alameda County every year. Our goals in doing so are to:


  • Provide direct health services & education to our residents
  • Connect residents with Alameda County resources, services, and programs
  • Understand the health needs of our residents, and act upon those learnings


Please submit a completed Health Fair Participation Request Form at least 30 days before the event date to be considered. We will make every effort to support your event, however, please note that not all requests for participation can be filled. *Please do not submit this form from a mobile device, it will not work.

Please let us know if you want educational materials, vaccines with optional health screenings, or any specific programs at your event (e.g. nutrition services, dental health, care partners with older adults, behavioral health, tobacco control, violence prevention, etc.). Please visit our website for a complete list of programs. Note: not all programs have the ability to attend events.

Are you requesting Immunization Services?

If you are requesting Immunization Services to be present at your event (COVID-19 vaccine, flu vaccine, mpox vaccine, rsv vaccine), please complete this additional form Mobile Vaccination Clinics Interest Form. Please note: Wellness Screenings (blood pressure screening, diabetes screening, etc.) are only available as an add-on service to Immunization Services and cannot be offered without a medical professional onsite.

We would like to minimize duplicating our service offerings at your event, and will collaborate with other providers already attending the event.
