AUTORIZZAZIONE AL TRATTAMENTO DEI DATI / PRIVACY AGREEMENT

Autorizzo con la presente al trattamento dei dati da me inseriti per le sole finalità legate all'erogazione del servizio, per il quale detti dati verranno trasmessi all’ IFO - San Gallicano (IRCCS) che effettua test e vaccini, e per la spedizione in forma telematica tramite cellulare o mail da me indicati del referto del Test effettuato.

Dichiaro di aver letto e compreso le informazioni fornite dal Titolare del trattamento a norma del Regolamento UE n. 679/2016 – GDPR di cui alla sopra estesa informativa e consapevole, in particolare, che il trattamento dei propri dati ha le finalità di cui all’articolo 2 della sopra estesa informativa sottoscritta ai sensi dell’art. 13 del GDPR (https://www.binario95.it/privacy/). Presto libero, consapevole, informato e specifico consenso al trattamento, anche con strumenti informatici e/o telematici, dei miei dati personali.

I hereby authorize the processing of the data entered by me for the sole purposes related to the provision of the service, for which said data will be transmitted to the IFO - San Gallicano (IRCCS) which carries out tests and vaccines, and for shipping in the form telematics via mobile phone or email indicated by me of the test report carried out.

I declare that I have read and understood the information provided by the Data Controller pursuant to EU Regulation no. 679/2016 - GDPR referred to in the above extensive information and aware, in particular, that the processing of your data has the purposes referred to in Article 2 of the above extensive information signed pursuant to art. 13 of the GDPR (https://www.binario95.it/privacy/). I will soon be free, aware, informed and specific consent to the processing, even with IT and / or telematic tools, of my personal data.
