Request for Voluntary Withdraw of Accreditation

CAAHEP accreditation is voluntary and a program is considered for accreditation only at the written request of the program sponsor. A program sponsor may also request voluntary withdraw of accreditation at the written request of the chief executive officer of the program sponsor and by submitting this form. Please review the full Voluntary Withdraw of Accreditation policy in the CAAHEP Policies & Procedures Manual (Policy 208.C.).


Any student who completes a program that was accredited by CAAHEP at any time during their matriculation is deemed by CAAHEP to be a graduate of a CAAHEP-accredited program. These students must be provided all the instruction promised by the institution and the institution is expected to maintain the academic integrity of the education program until these students have completed the program.


As of the effective date of voluntary withdraw of accreditation, the program will no longer appear on the CAAHEP website’s listing of accredited programs.

  • Continued Operation: the date should be within the program's current accreditation cycle. If it falls after the program's next scheduled comprehensive evaluation, a plan for withdraw must be submitted.
  • In Lieu of Negative Action: the date must be the same as the meeting date on which the CAAHEP Board would have considered the recommendation.
  • Planned Closure: the date should be set as the day after graduation of the last students.


Please note: The effective date of the voluntary withdrawal is the date the request is processed by CAAHEP or the date requested by the institution, whichever is later.

In accordance with CAAHEP Policy 208.C. programs may request voluntary withdraw of accreditation. In the instance where the criteria for the request for withdraw is Planned Closure, CAAHEP requests that the program submit a teach-out plan.


If the program or its Sponsor has an existing teach-out plan approved by its Institutional Accreditor, it should be submitted here. Programs that do not have an approved teach-out plan may submit a teach-out plan document that describes how the Program Sponsor will ensure that the delivery of education, training, and services to the remaining students, if any, will not be disrupted or modified and that the students will receive the remaining portion of the program in accordance with the terms set forth in the students’ enrollment agreement.

  • Payment of CAAHEP and Committee on Accreditation (CoA) current and outstanding fees.
  • Submission of all required and requested reports to the appropriate Committee on Accreditation.
  • Informing all current and potential students of the voluntary withdraw of accreditation and its effective date.
  • Deletion of all references to and claims of CAAHEP accreditation from websites, catalogs, advertisements and promotional materials.
