2024-2025 Unified Champion Schools (UCS) Intent Form


Upon completion, schools and community partners are able to receive any available resources from Special Olympics Nebraska, including funding for activities.


School requirements:

  1. Identify at least one building liaison. They will be the main point of contact and will receive information from Special Olympics Nebraska through newsletters and disseminate to the school, guide school in implementation strategies and activities, and collect/report data to Special Olympics Nebraska and Boston UMass.
  2. Schools must implement at least ONE activity in ONE of the three (Unified Sports, Inclusive Youth Leadership, or Whole School Engagement) during the school year.
  3. Follow Special Olympics Nebraska marketing guidelines.

Students with intellectual and developmental disabilities. These students generally need additional instruction/assistance with daily living skills, social skills, communication, safety awareness, learning, etc.

Please list only one person in this box who will be the primary point of contact for Unified activities in your school or site. Additional liaisons will be listed below.

Identify one other point of contact from the primary. This is necessary for us to ensure information gets to the school in instances of illness, medical leave, resignation, etc.


Inclusive Activities:

Select the Unified Sports your school/program plans to participate in this school year. Competitive Unified Sports are those that have a 50/50 ratio of athletes (students with intellectual disabilities) to partners (typically developing peers). Recreational Unified Sports are recommended, but not required to meet this ratio. If you are unsure of coach information at this time, please enter AD's information as a point of contact.

Select the Inclusive Youth Leadership activities your school/program plans to participate in this school year. Activities are considered leadership opportunities if your students help plan, execute, and clean up activities.

Select the Whole School Engagement activities your school/program plans to participate in this school year. Activities are considered Whole School Engagement if 75% of your student body participates.

Unified Courses are those that have a 50/50 ratio of athletes (students with intellectual and developmental disabilities) to partners (typically developing peers) in the classroom.


Commitment and Certification

Special Olympics Nebraska's Unified Strategy for Schools is a proven and effective strategy for creating a school climate of inclusion, acceptance, and respect for all students. By signing this form, the school administrator and liaison certify that:


  1. The school will work to implement components listed previously.
  2. The school liaison will report inclusive activities data information.
  3. The school liaison will complete a survey administered by the University of Massachusetts at the end of the school year.
  4. The school will seek approval for any Special Olympics branded materials (e.g., T-shirts, posters, etc.)
  5. Special Olympics Nebraska may use images, quotes, and videos from school events to promote the Unified Strategy for Schools.
