Pharmapack Health Product Awards 2022 - Entry Form

Should your product win an award, we will include this product description in external event communications. Please ensure that you are happy for any information given here to be shared publicly.

For the Jury to assess your entry, you must demonstrate how your product is innovative or displays a competitive advantage. Please use this space to share data, case studies, testimonies or use cases. Please tell us how many end have used your product? Were patients involved in the design and development? You may also upload content under 'Supporting Documents'.

Please select whether this is a new product, or a modification or evolution of an existing product

Please indicate the date your product has received market authorization from a Health Authority (must be January 2021 onwards)

Please specify in which countries or regions your product is for sale, or will be for sale in future.

What materials were used to create your product? Where is it manufactured? Please provide as much information as possible about materials, material reduction, composition, size and weight of your product. Crucially, please also include sustainability measures, or steps taken to reduce carbon impact.

You must supply images of your product, your company logo and most importantly a demonstration video. Entries without this information will not be considered. You can upload multiple documents one after the other, or send them together as a zip file including: 1. One image of your product in high definition (at least 300x300) 2. Your company logo as a vector file (ai or EPS) 3. A video demonstration of your product or solution. This could be either a marketing video, a case study, or a demonstration by one of your employees.
