Eligibility Requirements:


  1. Must address at least one of CCHA’s focus areas:
  2. Behavioral and mental health services
  3. Health screenings
  4. Dental hygiene care
  5. Specialty care services (e.g. diabetes)
  6. Patient advocacy and navigational resources
  7. Must serve indigent Chatham County, Georgia residents
  8. “Indigent” is defined, for this purpose, as households at or below 138% of the Federal Poverty Line in alignment with the U.S. Department of Health  and Human Services. The current income level for this funding cycle is equal to $20,783 per person or $43,056 for a family of four.
  9. Must provide documentation to certify client household income and location.
  10. Organization/entity must be a 501(c)(3) nonprofit or for-profit business with accompanying business licensing and certifications.
  11. Organization/entity complies with the USA Patriot Act and all federal and state laws concerning discrimination based on age, race, religion, gender or gender identity, sexual orientation, economic status, disability, cultural heritage or national origin.
  12. For-profit entities must provide:
  13. Most recent tax return;
  14. Last completed income statement (profit & loss);
  15. Balance sheet;
  16. Last completed audit, compilation, or review.
  17. Nonprofit entities must provide:
  18. Most recent IRS Form 990 or 990EZ;
  19. Last completed income statement (profit & loss);
  20. Last completed audit, compilation, or review.
  21. Organization/entity complies with all CCHA Indigent Care Trust Fund eligibility requirements and CCHA bylaws.
  22. Organization/entity has a conflict-of-interest policy.
  23. Grant funds cannot be used for capital campaigns or fundraising events.
  24. As a condition of accepting funding, Grantees must report data and outcomes through a mid-year and final report.
