YOUTH MINISTER/CAMPUS MINISTER/PASTOR RECOMMENDATION FORM

Please fill out this recommendation form completely. This information is critical to this youth being accepted to DYLC. A parish representative must participate in the Parish Contact Day which is the Thursday of DYLC (June 26, 2025, 9 AM – 4 PM).

IMPORTANT INFORMATION REGARDING PAYMENT:

No deposits are necessary for the 2025 DYLC.

If the Knights of Columbus, grandparents, or others are helping to financially sponsor DYLC attendees, they should write a check to the parish of the attendee and the parish will pay the Diocese.

Parishes will collect payment from each individual attendee. Once all payments have been collected the parish will send one check for the total amount of all attendees to the Diocese of Lansing.

• Payment of $450.00 per attendee is due by 03/19/2025
• Late payment of $500.00 per attendee is due 04/23/2025
