Instructions for Completing Intake Forms

Complete the following information for the person seeking services with StarCare Specialty Health System. If a question is not applicable, enter “N/A” in the space provided.


Completing this information ahead of time will shorten the time needed to complete your intake. Upon completion of the intake process, if eligible, you will be authorized for services based on your need. The documents listed below are necessary to complete the intake process. The length of time required for intake could be reduced by uploading and submitting the required documents below. Originals will need to be brought the day of intake/Open Access for verification purposes.


  • Proof of residency
  • Proof of household income
  • Proof of Insurance, if applicable
  • Proof of guardianship or conservatorship, if applicable
  • Proof of identity with picture (driver’s license, permit, state ID, passport)
  • Social Security card
