Mandatory Meal Plan Appeal Form

For students seeking an exemption or adjustment to a required university meal plan.

Mandatory Meal Plan Appeals

First-time freshman students who are required to live on campus must participate in a university meal plan, per the housing contract. Appeals will be considered only for documented medical conditions, religious dietary restrictions, or significant special circumstances (such as verified financial hardship).

Submission of this form does not guarantee approval. All appeals must include appropriate documentation and will be reviewed by a committee before the fee payment deadline. If appropriate, an alternative meal plan option may be offered.

Term for Appeal*

Eligibility

Meal Plan Appeals will be considered for the following reasons:

Medical Conditions: Students with medical conditions that require dietary restrictions not accommodated by dining services must provide documentation from a licensed medical professional.

Religious Dietary Restrictions: Students following religious dietary guidelines that the available meal options cannot meet must submit a statement from a religious leader.

Other Special Circumstances: Appeals based on financial hardship will only be considered for students facing significant, documented challenges, such as homelessness or extreme financial distress. Students must be actively working with Student Accounts and Billing Services, UWG CARES Team, or another designated department to address their specific situation. Appeals based on general financial concerns or a desire to reduce costs will not be considered.

Reason for Appeals Request*

Please provide a detailed explanation of your reason for appealing the meal plan requirement. Include how your circumstances affect your ability to participate in the campus meal plan.

Required documents for each type of appeal:

Required Documentation

You must attach the appropriate documentation based on your appeal type:

Medical Condition: Documentation from a licensed medical professional stating your diagnosis and dietary needs.

Religious Restriction: A signed statement from a religious leader outlining your dietary restrictions and why they cannot be accommodated.

Financial Hardship / Special Circumstance: A letter or documentation from Account Services or another campus official verifying financial hardship, housing insecurity, or other special circumstances.
