This request form can be used by ASU employees, student workers, or registered volunteers who need to expedite the entering of their Motor Vehicle Record check date and/or their authorized driver training completion date.

EXPEDITE REQUESTS SHOULD ONLY BE USED IN ONE OFF URGENT SITUATIONS WHERE EXPEDITED AUTHORIZATION IS ABSOLUTELY NECESSARY SO THAT THE DRIVER CAN PERFORM THEIR DRIVING DUTIES.
