Intent to Apply Form
This form serves as a preliminary step in the grant application process, helping us determine alignment between applicants’ proposed projects and our funding priorities. Selected organizations will be invited to submit a full grant application.
We provide grants ranging from 75K to 300K per year (awards at the 300K level are rare). Each year, we fund approximately 12-15 organizations whose projects align with our priorities.
This form will close on May 15th, 2025 at 5 pm.
Before proceeding, please read the information below.
ELIGIBILITY REQUIREMENTS
• Applicants must be a Community Based Organization (CBO) with a demonstrated history of serving children, families, and/or expectant mothers within our defined service area in California: Santa Clara, San Mateo and/or Santa Cruz counties.
• Organizations must hold 501 (c)(3) nonprofit status or have a fiscal sponsor with such status.
• Projects must address at least one of the top three health priorities identified in our most recent Community Health Needs Assessment (CHNA):
- Economic Stability
- Youth Mental/Behavioral Health
- Healthcare Access
• Applicants must demonstrate how their project impacts underserved children and families.
• For projects involving multiple stakeholders, the lead applicant must be a CBO and provide evidence of collaboration with other entities.
• Strong applicants serve communities within our service area with a focus on those low or very low Child Opportunity Index (COI) areas.

1. Second Harvest: Alleviating hunger for families in Silicon Valley.
2. Puente de la Costa Sur: Supporting economic security and health access for rural families and children.
3. Adolescent Counseling Services: Expanding youth mental health services.
